We’re going to create a simple form using their template library, nothing too advanced. But it will give you an idea of the platform interface.

Let’s get started.

Step 1: Click on the “Create Form” option on the JotForm dashboard.

On the left-hand side of the screen, you can see all the template types listed together. We’re going to design a feedback form for this review.

We’re impressed by the number of options they have. In total, they have 771 templates within the Feedback Forms category.

As you can see in the screenshot below, there is a direct option for it. Also, it is worth noting that you can add your own logo too by clicking the option at the top of the form.

Step 6: We want to add the field at the top of the form. To do that, select the field before it. We selected the “Event Feedback” box since we want the category placed under it.

Step 7: We are quite happy with the other fields given in the form template. So, we’ll let it be as it is. To create the form, we clicked on the “Publish” option at the top.
